Abstract

|
목적: 본 연구는 한국형 소아 환자 분류도구(KPPCS)의 신뢰도와 타당도를 검증하고 간호시간 환산지수를 산출하기 위해 수행되었다.
방법: 연구는 다양한 지역과 병상 규모를 포함한 9개의 어린이병원에서 수행되었다. 소아 환자 분류도구의 조사자 간 신뢰도를 구하기 위해 21개 병동의 간호관리자와 간호사가 시행한 575명의 환자분류 자료를 분석하였고, 구성타당도 검증은 환자분류 조사 당일의 환자 집단의 나이, 재원일수, 재원 형태(재원 중, 응급실 입원, 외래 입원, 퇴원), 진료과 등을 집단 비교법을 이용하여 검증하였다. 575명의 환자에게 제공된 직접 간호시간과 간접 간호시간은 284명의 간호직원들이 하루 동안 스톱워치를 이용한 자가보고를 통해 측정되었다.
결과: 개발된 한국형 소아 환자 분류도구는 12개 영역, 55개 간호활동, 80개 항목과 적용지침으로 구성되었다. 조사자 간 신뢰도는 일치도가 높은 것으로 나타났으며 (r=.91, p<.001), 연령과 재원일수, 재원 형태와 진료과별 환자분류점수가 통계적으로 유의한 차이가 있는 것으로 확인되어 구성타당도가 검증되었다(p<.05). 간호시간과 환자분류점수 또한 통계적으로 유의한 상관관계를 보였고(r=.59, p<.001), 환자분류점수 1점당 간호시간 환산지수는 10.78분으로 나타났다. 최종 KPPCS 환자분류군은 4개군으로 분류되었다.
결론: 본 연구를 통해 KPPCS는 소아 환자의 복합성에 대한 간호 요구도가 반영된 유용한 환자분류 도구로 사용될 수 있을 것이다.

Purpose: This study was performed to verify reliability and validity of the Korean Pediatric Patient Classification System (KPPCS) and estimate nursing time conversion index.
Methods: The study was conducted in 9 children’s hospital which included various areas and size of bed settings. To verify intraclass correlation reliability and construct validity, staff nurses and nurse managers of 21 wards classified 575 patients according to KPPCS comparing differences by age, days of stay, type of stay and medical department. Direct and indirect nursing time of 575 patients were measured by 284 nursing staffs by stopwatch observation and self reports for 24 hours.
Results: KPPCS has 12 categories, 55 nursing activities and 80 criterions. High agreement among nurses (r=.91, p<.001) suggested substantial reliability. Construct validity was verified by comparing differences in age, days of stay, type of stay and medical department (p<.05). The correlation of nursing time and classification score was also statistically significant (r=.59, p<.001). The nursing time conversion index was 10.78 minutes per 1 classification score. The entire patient group were classified to four groups using KPPCS.
Conclusion: The findings suggest that KPPCS would be a useful tool for estimating nursing demands related to the complexity of pediatric patients.
